Shoulder instability occurs when the shoulder joint is unable to maintain normal control during movement, leading to feelings of slipping, subluxation, or dislocation. It commonly affects younger athletes but can occur at any age following trauma or repeated strain.

What is shoulder instability?

The shoulder is the most mobile joint in the body, relying heavily on muscles, ligaments, and the labrum for stability.

Instability may be:

  • Traumatic (following a fall or collision)

  • Atraumatic (gradual onset or related to hypermobility)

  • Recurrent (repeated episodes of giving way or dislocation)

Common symptoms

  • Sensation of the shoulder slipping or giving way

  • Pain following a dislocation or subluxation

  • Apprehension with certain movements

  • Reduced confidence using the arm

  • Weakness or loss of control during activity

Rehabilitation focuses on:

  • Rotator cuff and scapular strengthening

  • Neuromuscular control and joint awareness

  • Gradual exposure to unstable or high-demand positions

  • Sport- or work-specific movement retraining

Programs are individualised based on injury history, demands, and goals.

Do I need imaging or surgery?

Imaging may be required following traumatic dislocation. Surgery may be considered for recurrent instability, but physiotherapy remains critical both before and after surgical intervention.
